St. Joseph’s Hamilton implements system in ICU

HAMILTON, Ontario – St. Joseph’s Healthcare Hamilton (SJHH) announced their Critical Care Program Level 3 Intensive Care Unit (ICU) has implemented a new electronic documentation system, Intellivue Clinical Information Portfolio (ICIP). The system provides comprehensive patient information from health professionals, nursing and respiratory therapists in real-time for prompt decision making.

Automatic transmission of patient demographics from the Hospital ADT system to Philips electronic documentation and patient monitoring systems improves data integrity and patient flow. As a critical patient enters into care of ICU clinicians, staff can begin customizing the screens to meet the patient’s unique requirements. Bedside monitors send a multitude of hemodynamic parameters to the review screens for validation.

Critical ECG wave forms are captured and available in the patient record for trending. Programmed intravenous drips calculate mcg/kg/min recording each change in rate and volume with automatic totals. Ventilators download their information directly to ICIP thereby reducing manual intervention and chance of error.

The system assists clinical staff in achieving patient safety indicators. A message bar notifies staff of incoming lab results while flagging those that require immediate attention. Wound photographs can be embedded with assessment documentation. Clinical Advisories prompt staff to document bundle intervention completion to meet best practice and patient safety standards in alignment with the SJHH organization and Safer Healthcare Now! Canada Ventilator Associated Pneumonia and Central Line Associated Blood Stream Infection Prevention initiatives.

Each of these tools enables Nurses and Respiratory therapists working at the bedside to ensure their standards of practice are achieved for each individual patient, by providing alerts and monitor indicator reminders in the system.

The Patient Summary displays a snapshot of the most recent two sets of lab results, vital signs, respiratory and fluid status on one screen. All current and previous ICU encounters are available online from any bedside device, PC, or remotely with VPN access. ICIP Critical Care automatically calculates scores, such as SAPS II, TISS 28, APACHE ll, Braden, Harris Benedict and the Glasgow Coma Scale, based on routine parameters in patient records. SJHH has customized specialized screens for each discipline allowing for filtering of critical elements relevant to practice.

“Simply having a computerized information system does not guarantee improved patient care, in fact having too much information can lead to errors that directly impact patients due to the excessive cognitive load required to pick out the important information and to integrate it into our clinical thinking,” said Dr. Mark Soth, Head of Service Critical Care Service SJHH. “ICIP allowed us to customize views to give us only the information we need in a format intuitive for our clinical thinking that minimizes this. A simple marker for a good electronic clinical solution is that it should feel like it is working for you and not the other way around.”

“Critical to the successful implementation of ICIP in ICU was the multidisciplinary engagement and information sharing at all levels. Establishment of an ICIP Implementation team provided a framework for system development and an opportunity to engage front-line staff as Champions,” said Angela Greiter, Charge Nurse Intensive Care Unit. “Configuration of the application by the ICIP Champions, according to the needs of the patients served in the ICU, was a considerable undertaking and testing was conducted at every stage. Training needs were significant and multi-faceted and training was provided by front-line staff.”
